Choosing the right service
If you already know what you need, go straight to the service below. If you do not, that is normal. Tell us the young person age and what a difficult day looks like, and we will point you to the right route, including telling you when it is not us.
Children's outreach and home support
Support workers in the family home and out in the community. Personal care, help with routines and communication, activities, after school support and school holiday cover. Short breaks that give parents room to breathe.
Best for: children living at home whose families need practical support, and children with additional needs who benefit from one to one time.
Funded through the local authority, direct payments, a personal budget, or privately.
Supported accommodation for 18+
Your own place, with support built around what you actually need help with. Budgeting, cooking, shopping, appointments, benefits and paperwork, work and study, and keeping the tenancy going.
Best for: young adults aged 18 and over, including care leavers, moving toward living independently.
Support hours flex as skills grow. The aim is for you to need us less.
Learning disability and autism support
Person centred support delivered wherever the person lives, at home, in the community or in supported accommodation. We work with the communication the person already uses, whether that is speech, Makaton, symbols or a communication app.
Best for: children and young adults with a learning disability or autism who need consistent, well matched support.
Complex needs support
One to one and two to one support, personal care, mobility and transfers, communication support and behavioural support, for young people whose needs are high and whose day needs careful planning.
We provide personal care and daily living support. Clinical and nursing tasks remain with the health professionals responsible for them, and we work alongside those teams rather than in place of them.
Best for: children and young adults with high support needs living at home or in supported accommodation.
Transitions to adult services
Turning eighteen should not mean starting again with strangers. We plan moves from children's services into adult support gradually, building the skills first and keeping familiar staff involved where we can.
Best for: young people aged 16 and over preparing to move into adult support.
